Health Risk Assessment

At MedCost Benefit Services, the core component of our integrated health and wellness program is the Succeed™ health risk assessment (HRA) that identifies health behaviors and associated lifestyle challenges. Available through MedCost's partnership with HealthMedia, the unique technology and proven behavioral science of the HRA are used to identify a person’s top four behavior targets, barriers to change, social support and core issues, as well as willingness, readiness, and confidence to change. This technique helps to create behavior change interventions tailored to each participant.

Data from the Succeed™ HRA provides a snapshot of employees’ current health status…as well as their main health concerns. Based on HRA results, members may be referred to personalized lifestyle management or behavioral health programs. There are four web-based lifestyle programs available:

  • Breathe™ (smoking cessation)
  • Relax™ (stress management)
  • Nourish™ (nutritional counseling)
  • Balance™ (weight management)

Members also can participate in three behavioral health programs:

  • Overcoming™ Insomnia
  • Overcoming™ Binge Eating
  • Overcoming™ Depression

Based on specific triggers within the health risk assessment (HRA), members also may be referred to the Personal Care Management program for further evaluation as a potential candidate for nurse coaching. As part of this evaluation, a nurse reviews claim and prescription data, the member’s responses to the HRA, and any communications that have been provided to the member based on HRA results. This complete evaluation ensures continuity throughout all of MedCost’s health and wellness programs.
